Case management is a collaborative process. We assess, implement, monitor, and evaluate plans to help each individual and family meet the goal of self- reliance. Mid-KS CAP’s work is characterized by advocacy, communication, and resource management that promotes personal accountability. Case management services in the Higher Achievement Together (HAT) program are designed to use the appropriate funding streams to address services for the individual or family to remove obstacles and solve problems that block the achievement of self-reliance.
Does it work? Absolutely. Mid-Kansas CAP serves hundreds of people each year who take the resources we give them and use them to overcome even the most difficult circumstances. What makes case management special is the same spirit of independence that flows through all Community Action Agencies. No client will ever go through a training “just because”. Everything is done with a purpose to meet the needs of a specific person, family, or community. The struggles of a mom in Augusta are different than a student in Wellington, just like poverty in Kansas looks different than poverty in Texas or Oregon. We understand that people come from different backgrounds, and we work WITH our customers (not FOR them), in their current situation, to help each reach their full potential.
